- Available online, at the Embassy entrance, or at Friendship Tours
- Fill out all items correctly, use [N/A] if not applicable
- Size: 4.5cm x 3.5cm with white background
- Take it within 6 months, and write your name and birthdate on the back.
- Paste on the application form
- Issued within 1 year from PSA
- Include additional documents for late registration
- Issued within 1 year from PSA
- Include "Certificate of Non-Record" if no PSA record
7. Bank Certificate with Average Daily Balance for the last 6 months
8. Income Tax Return (Form 2316)
9. Employment Certificate indicating period, salary, position
For students: Copy of school ID or certificate of Enrollment
Additional Requirements: If the Bank Certificate and Tax Return are unavailable, provide a Bank Statement for the last 6 months.
10. For Guarantor: (someone else will shoulder applicants expenses in Japan)
- Guarantee Letter
- Proof of relationship
- Bank Certificate and Income Tax Return of the Guarantor
- Guarantor’s Employment Certificate
